Pencil cup as toothbrush holder
The Hengsts have a solution for those fat, character toothbrush handles that don't fit into the holes of a regular countertop toothbrush holder:
We have the problem in our house that kids' toothbrushes don't fit in the traditional toothbrush holders because of all the character handles, music playing handles, etc. So we improvised and use a desktop pencil holder, similar to the one shown. They have dividers in them so that the toothbrushes can still be separated.
When it's time to clean, I just use a washcloth over the end of a butter knife to clean out the corners (although I suppose you could use a bottle brush also). No they're not pretty, but certainly do serve the purpose.
Re: cleaning -- I just stick mine in the top rack of the dishwasher.

re: cleaning of the toothbrush holder.
This might not be great for those who have really little kids, but we use a vintage jam jar and pour a little mouthwash in the bottom of it.

We use a thick plastic Backyardigans cup that has a weighted bottom so it doesn't tip over. This way, DS gets to have his favorite TV show characters, even if I can't find any Backyardigans toothbrushes!

We use a wide-mouthed ceramic pot from Ikea. If I remember correctly, it was only $0.75. And it's easy to clean (just handwash or stick in the dishwasher).

We now use our old toothbrush holder for the kids' paintbrushes after washing and between uses. It's nice and heavy, so it doesn't tip over, even with the tall brushes in it!
